PADI Forms

Here are some links to different resources PADI has available online. 

• Lost Card
  Lost your certification card?  Here's the link to the PADI website to get it replaced, either online or through the mail.
• Medical Statement
  To participate in any of our classes, we must have a current version of this Medical Release.  If you answer 'yes' to any of the items on this medical release, please contact us, and your doctor, prior to starting your class.
